Before we can start your card replacement we need to know more about your card.

Card Type

Please indicate the type of your card by matching the serial number on the back of your card with the pattern that is circled on the images below.

Card Type 1 has a serial number with up to nine digits and a lot number consisting of letters and numbers.
Card Type 2 has a serial number with up to nine digits, a one-digit check sum, and a lot number consisting of letters and numbers.
Card Type 3 has a lot number consisting of letters and numbers and a serial number with sixteen digits.

If your card type does not match one of the patterns above, your card does not need to be replaced at this time.
